Description

(Sterile Powder for Injection – Broad-Spectrum Third-Generation Cephalosporin Antibiotic)

High-Potency Injectable Antibiotic for Severe Bacterial Infections

Ceftriaxone 1 Gram Vial is a sterile, broad-spectrum, third-generation cephalosporin antibiotic used for the treatment of a wide range of moderate to severe bacterial infections. Presented as a powder for reconstitution, it is designed for intravenous (IV) or intramuscular (IM) administration under the guidance of a licensed healthcare provider. This formulation offers excellent bactericidal activity, high tissue penetration, and an extended half-life that allows for once-daily dosing in many indications.

Ceftriaxone is marketed under the brand name Rocephin and is recognized globally as a first-line treatment for serious infections including pneumonia, sepsis, meningitis, urinary tract infections, intra-abdominal infections, and certain sexually transmitted diseases (STDs) such as gonorrhea.

Product Specifications

  • Form: Sterile lyophilized powder for injection

  • Strength: 1 gram per vial

  • Route of Administration: IV or IM injection after reconstitution

  • Packaging: Single-use vial

Mechanism of Action

Ceftriaxone works by inhibiting bacterial cell wall synthesis through binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. It exhibits bactericidal activity against a wide spectrum of Gram-positive and Gram-negative bacteria, including many beta-lactamase–producing strains.

Its extended half-life (6–9 hours) permits once or twice daily administration, reducing dosing frequency and improving compliance in both inpatient and outpatient settings.

Spectrum of Activity

Ceftriaxone is effective against:

  • Gram-negative: Escherichia coli, Klebsiella spp., Neisseria gonorrhoeae, Haemophilus influenzae, Proteus spp., Enterobacter spp., Salmonella, Shigella

  • Gram-positive: Streptococcus pneumoniae, Streptococcus pyogenes, MSSA (methicillin-sensitive Staphylococcus aureus)

  • Anaerobes: Limited activity; often combined with metronidazole for broad anaerobic coverage

Not effective against: MRSA, Enterococci, Pseudomonas aeruginosa (limited)

Indications

Ceftriaxone 1g is used for treating:

  • Community-acquired and hospital-acquired pneumonia

  • Meningitis and other CNS infections

  • Sepsis and bacteremia

  • Complicated urinary tract infections (UTIs)

  • Gonorrhea (including multi-drug resistant strains)

  • Skin and soft tissue infections

  • Intra-abdominal infections (usually in combination with metronidazole)

  • Post-operative infections

  • Prophylaxis in certain surgical procedures

Dosage and Administration

  • Adult dosage: Typically 1–2 grams once daily; up to 4 grams/day in severe infections

  • Pediatric dosage: 50–100 mg/kg once daily depending on severity and indication

  • IV use: Reconstitute with sterile water or appropriate IV fluid (e.g., D5W, 0.9% NaCl); administer over 30 minutes

  • IM use: Reconstitute with 1% lidocaine (not for IV use); inject deep into a large muscle mass

  • Duration: Determined by infection severity and clinical response

Always reconstitute and administer according to standard aseptic guidelines. Do not co-administer with calcium-containing solutions in neonates.

Safety and Tolerability

Ceftriaxone is generally well tolerated, but as with all antibiotics, monitoring for adverse reactions and signs of allergic response is essential.

Common side effects:

  • Injection site pain (IM), phlebitis (IV)

  • Diarrhea or mild gastrointestinal upset

  • Skin rash, eosinophilia

  • Transient elevation in liver enzymes

Rare but serious reactions:

  • Anaphylaxis in penicillin-sensitive patients (cross-reactivity <10%)

  • Hemolytic anemia

  • Clostridioides difficile–associated diarrhea (CDAD)

  • Biliary sludging or gallbladder pseudolithiasis (especially in neonates and prolonged use)

Warnings and Precautions

  • Hypersensitivity: Contraindicated in patients with known allergy to cephalosporins

  • Neonates: Avoid use with calcium-containing IV solutions due to risk of precipitation

  • Renal/hepatic impairment: Adjust dose based on clinical judgment, although ceftriaxone is dual-eliminated

  • Monitor for superinfection, especially with prolonged use

Storage and Handling

  • Unreconstituted vials: Store at room temperature (15–25°C), away from moisture and light

  • Reconstituted solution: Use within 24 hours if refrigerated (IV); 6 hours at room temperature

  • Do not freeze reconstituted product

  • Use sterile technique for preparation and administration
